USPS FORM 1583 Instructions


Anyone considering third-party mail servicers to handle their mails or packages is required to complete the USPS Form 1583. This form allows the third-party company to process and handle mail on the filler’s behalf. The third-party is known as a Commercial Mail Receiving Agent or CMRA for short. USPS Form 1583 is applicable to both businesses and individuals.

USPS FORM 1583 Instructions

Sections 1 and 2 are to be completed by the person or authorized business representative that is requesting CMRA services. The third-party mail servicer or CMRA completes section 3.

  • Box 1: Enter the date when you are filling this form
  • Box 2: Enter your name (individual) or business representative’s name (business)
  • Box 3: Enter the address provided by Brield
  • Box 4: Leave blank
  • Box 5: Sign the form
  • Box 6 & 7: If additional party/parties will be sharing the assigned address, include their information here
  • Box 8: Mark which IDs are being submitted for verification. One of the IDs is required to be a photo ID (for businesses, ID must belong to the authorized business representative)
  • Box 9: Applicable for business customers only
  • Box 10a-10e: Provide the authorized agent’s information
  • Box 11: Specify the industry the business operates in
  • Box 12: If additional business entities are using the address, their information must be provided in this section.


USPS Form 1583 requires to be notarized. After the form is completed, it must be scanned along with your IDs and forwarded to Brield. We will complete section 3 at that time and submit on your behalf.


  • State or federal government ID, this includes driver’s licenses
  • Motor vehicle registration
  • Mortgage, deed or lease
  • Passport or Naturalization certificate
  • Voter ID
  • Insurance policy declaration page issued to the individual or the business


Customers cannot use birth certificates or Social Security cards for verification purposes.
